• Proof of liability insurance
• Copy of Wisconsin Sellers Permit
• Copy of Food Permit from the Wisconsin Dept. of Health and Family Services
• Proof of non-profit organization’s non-profit status (if applicable)
• Signed copy of Policies & Procedures document

• The vendor fees are non-refundable.
• All vendors will need to be approved by committees.
• If accepted, vendor fees are $50 per 10′ x 10′ space, per event, to be paid prior to the event.
